Required documents for application:

Applicants are required to provide the following documents:

1- A written letter explaining the health, social and financial status
2- Medical report with treatment plan and the estimated cost (recent report).
3- Salary certificate from the workplace of the patient or primary income earner (or equivalent).
4- Bank statement for the last 6 months (or equivalent).
5- Lease agreement (if the person is not the owner of the residence).
6- Copy of the passport, residency, and ID card of the patient, the primary income earner and the rest of his family.

Note: Upon receipt of all the required documents, the file will be sent to the relevant department and you may be asked to submit additional documents to complete the application.
